About the role

Provide mentorship and guidance to individuals recovering from substance use disorders using lived experience. Manage a caseload, facilitate support groups, and connect clients with essential community resources to sustain sobriety.

Position Summary

Provides support, guidance, and mentorship to individuals in recovery from substance use disorders. Drawing from their own lived experience, help clients navigate recovery resources, develop coping strategies, and connect with support networks. Participate in outreach to community partners to help coordinate services for clients to help sustain sobriety. Maintain regular contact with clients to monitor and assess progress.

Position Description

Manage a caseload of peers in the community or program settings. Track and assess progress as determined by program Assist peers in developing treatment plans that personalize recovery goals. Efficiently operate personal computer to enter, edit and/or retrieve data using agency electronic health record

Assist clients in accessing community resources, including, but not limited to: food, clothing, supportive housing, Medicaid, job readiness and/or placement, public assistance, etc.

Facilitate site-based daily support/peer groups

Visit, support and transport clients as needed as determined by program Collaborate with family members, internal and external case managers, and other providers while supporting client’s recovery

Prepare documentation in compliance with contract, regulatory and Agency requirements

Other duties as assigned

Required

Knowledge, Skills and Abilities

  • Knowledge of substance use disorders and community resources to assist with maintaining sobriety
  • Effective verbal and interpersonal communication skills
  • Ability to operate personal computer to enter, edit and/or retrieve data and to produce reports and correspondence as necessary
  • Beginner level problem-solving skills and ability to multi-task while prioritizing tasks
