United Wound Care Centers is seeking a compassionate and clinically skilled Nurse Practitioner or Physician Assistant to join our growing wound care team. This position focuses on the evaluation, treatment, and management of acute and chronic wounds in both clinic and home-based settings.

The ideal candidate is patient-focused, detail-oriented, and comfortable working independently while collaborating with supervising physicians and other healthcare professionals. Prior wound care experience is preferred but not required for candidates with strong clinical assessment skills and a willingness to learn specialized wound management techniques.

Responsibilities

  • Perform comprehensive wound assessments, including measurement, staging, and documentation of wounds.
  • Develop and implement individualized treatment plans for patients with acute and chronic wounds.
  • Perform conservative sharp wound debridement within scope of practice and according to established protocols.
  • Evaluate patients for advanced wound care therapies, including skin substitutes, collagen products, compression therapy, and offloading devices.
  • Monitor wound healing progress and modify treatment plans as clinically indicated.
  • Order, review, and interpret diagnostic testing, laboratory results, and imaging studies relevant to wound healing.
  • Educate patients and caregivers regarding wound care, infection prevention, nutrition, and treatment compliance.
  • Maintain accurate and timely electronic medical records (EMR) documentation.
  • Collaborate with supervising physicians, nurses, home health agencies, and other healthcare providers to optimize patient outcomes.
  • Prescribe medications and wound care treatments within state regulations and collaborative practice agreements.
  • Identify and manage wound-related complications, including infection, vascular insufficiency, and delayed healing.

Qualifications

  • Current Delaware Physician Assistant or Nurse Practitioner license in good standing (or ability to obtain).
  • DEA registration preferred.
  • Minimum 1 year of clinical experience as a PA or NP preferred.
  • Wound care, dermatology, vascular, surgical, primary care, home health, or skilled nursing experience preferred.
  • Strong clinical assessment and diagnostic skills.
  • Experience with EMR documentation systems.
  • Excellent communication, patient education, and interpersonal skills.
  • Ability to work independently while maintaining close collaboration with supervising physicians and clinical staff.
  • Reliable transportation for travel to patient homes and partner facilities, if applicable.

Preferred Qualifications

Wound Care Certification (WCC, CWCN, CWS, or equivalent). Experience with skin substitutes, biologics, compression therapy, and chronic wound management. Experience providing care in outpatient, home-based, or long-term care settings. Requirements

Candidates must hold a valid Delaware NP or PA license and possess strong clinical assessment skills. Prior experience in wound care, dermatology, or vascular health is preferred, along with a DEA registration.
