About the role

Responsible for the full case management process, including assessing high-risk patients and developing comprehensive treatment plans. Coordinates the delivery of services and leads Transition of Care activities within an interdisciplinary team.

Job Summary

Responsible for the case management process including assessment/reassessment, service planning, and monitoring of Morton patients.

Essential

Duties and Responsibilities

  • Identifies, assesses, and works with high-risk patients
  • Conducts initial screenings to determine eligibility
  • Develops comprehensive treatment plans
  • Communicates with care team
  • Engages with patient to assure understanding of goals and assists in meeting goals, monitoring progress
  • Coordinates delivery of service
  • Leads Transition of Care (TOC) activities
  • Refers all identified social drivers of health to Community Health Worker or designee
  • Coordinates with interdisciplinary team and documents all services provided

Requirements

  • Current OK nursing (LPN or RN) license in good standing is required.
  • 2+ years of demonstrated clinical experience in healthcare.
  • 1+ years of demonstrated Clinical Case Management or Care Management experience is required.
  • Bilingual in Spanish preferred, but not required
  • Current BLS/CPR certification required.
  • Demonstrated ability to work effectively with staff and the public.

This position has been deemed a” Safety-Sensitive” position; therefore, Morton reserves the right to refuse to hire an applicant or may terminate an employee in a “Safety-Sensitive Position” who tests positive for medical marijuana even if he/she has a valid medical marijuana license.
