About the role

Provide compassionate end-of-life care to patients and emotional support to their families. Develop and implement comprehensive plans of care while collaborating with a multidisciplinary team to ensure optimal patient outcomes.

Hobart Medical Building 101 W 61st Ave Hobart, Indiana 46342 Our Hospice Nurses care for patients at the end of their lives. Their main responsibility is to help people live as comfortably and independently as possible and with the least amount of pain, in their final days. One of the largest parts of being a Hospice Nurse involves helping patients, and their families, feel more comfortable about death and providing them with the emotional support they need. Our RNs go beyond that and care for the patient’s mind, body and spirit while supporting their families and caregivers as well.

We have the ability to work with your schedule.

Determine patient eligibility for home care and hospice care.

Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.

Collaborate with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience.

Assess, collect, and analyze comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care.

Develop an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family, and continuously review and revise the plan of care as patient condition warrants.

Prioritize and implement the identified plan, coordinate care delivery, employ interventions, to improve patient outcomes while providing a safe, respectful, and ethical practice environment.

Communicate timely changes in patient’s assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care, and maintain and coordinate communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery.

Promote patient's independence by establishing patient care goals.

Serve in an on-call rotation and provides patient care outside of regular business hours.

Qualifications

Requirements

Requires an Associate's degree in Nursing, a valid RN license, CPR certification, and a driver's license with auto insurance. Experience in home health or hospice care and a Bachelor's degree are preferred.
