Licensed Practical/Vocational Nurse (LPN/LVN) – Home Health Job Summary We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.
Location: West Riverside
Provide nursing care according to the RN‑developed Plan of Care. Monitor and document changes in patient condition; report to RN/physician as needed. Assist with activities of daily living to promote patient independence. Educate patients and caregivers within LPN/LVN scope of practice. Maintain accurate EMR documentation. Participate in case conferences and coordinate care with the home health team. Follow infection‑control, HIPAA, and company policies.
Current LPN/LVN license in the state of practice. Graduate of an accredited practical/vocational nursing program. Minimum 1 year acute care experience preferred. Current CPR/BLS certification. Valid driver’s license, reliable insured vehicle. Strong communication, clinical judgment, and ability to work independently. Ability to pass required physical exams and annual PPD testing.
