Avivo is expanding its innovative housing continuum with the launch of Avivo Village Central Minnesota, creating new opportunities to support individuals experiencing homelessness as they move toward stability, recovery, and permanent housing. As a Case Manager, Housing (Site Based), you will be among the first team members helping shape the culture, services, and participant experience within this new community. This is more than a case management role—it's an opportunity to help build a program from the ground up while supporting individuals on some of the most important journeys of their lives. You'll work directly with residents to develop goals, strengthen stability, access resources, and create pathways to long-term housing success. Through relationship-based, person-centered services, you'll help individuals achieve greater independence while ensuring they feel seen, valued, and supported along the way. What You'll Do Build trusting, person-centered relationships with residents experiencing homelessness and other complex barriers Complete assessments and partner with residents to develop individualized housing and wellness goals Provide ongoing case management services focused on housing stability, recovery, and increased independence Support residents in accessing healthcare, mental health services, substance use treatment, benefits, employment, and community resources Coordinate services with internal teams, landlords, community partners, and service providers Help residents build skills related to housing retention, tenant responsibilities, and independent living Utilize Housing First, Harm Reduction, Motivational Interviewing, Trauma-Informed Care, and person-centered practices Assist residents in navigating challenges, resolving barriers, and responding to crises as they arise Maintain timely, accurate documentation and required reporting in electronic systems Contribute to a collaborative team environment focused on dignity, recovery, and positive participant outcomes QualificationsWhat You Bring High school diploma or GED One or more years of experience working with individuals experiencing homelessness or similar populations OR an associate degree or higher (no experience required) Lived experience may be considered toward experience requirements Passion for supporting individuals experiencing homelessness and housing instability Strong relationship-building skills and ability to engage people with compassion and respect Understanding of or willingness to learn Housing First, Harm Reduction, Motivational Interviewing, and Trauma-Informed Care Strong communication, organization, and problem-solving skills Ability to work effectively with individuals from diverse backgrounds and life experiences Knowledge of community resources or the ability to quickly learn available supports Ability to manage documentation and multiple priorities in a fast-paced environment Valid driver's license, reliable transportation, and ability to travel as needed What We Offer 32-hour full-time work week

Requirements
