About the role

The Discharge Planner is responsible for coordinating patient transitions and managing clinical cases within a behavioral health setting. The role requires high-level organization and the ability to multi-task to ensure effective patient care and healing.

Qualifications

  • Bachelor’s Degree in the human services field (counseling, psychology, social work) required, Master’s degree preferred.
  • Previous professional experience in a behavioral health setting preferred.

Requirements

A Bachelor's degree in human services, such as counseling, psychology, or social work, is required, while a Master's degree is preferred. Previous professional experience in a behavioral health setting is also preferred.
