About the role

The Acute Case Manager evaluates inpatient medical records using SI/IS criteria and facilitates discharge planning across acute, skilled, and emergency settings. They collaborate with physicians and care coordination staff to ensure appropriate utilization of hospital services and continuum of care.

High Desert Medical Group is seeking a full time Acute Case Manager who will be responsible for evaluating inpatient medical records using SI/IS criteria, communicating with patients and families, facilitate discharge planning needs for the patients in the acute setting, skilled setting, and emergency rooms.

Summary of Duties

  • Perform inpatient reviews at each facility daily where patients of HDMG are hospitalized in an acute setting, weekly in the skilled setting, and time of visit for appropriate ER patients.
  • Reviews medical record of each patient for SI, IS criteria.
  • Communicate with hospitalist, attending physician and specialists regarding any utilization of hospital service issues that are noted.
  • Communicates with care coordination staff regarding discharge planning needs.
  • Identifies and initiates discharge planning needs along with hospital case management staff.
  • Assist hospitalist and attending physician to identify continuum of care needs.
  • Facilitate discharge plans.
  • Attend Hospital Utilization Meetings and acute rounds as needed.
  • Participate in Case Manager's "on call" rotation.
  • Perform other duties as assigned.

The pay range for this position at commencement of employment is expected to be between $31.76 and $43.70/hourly, non-exempt; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.

If hired, employee will be in an “at-will position” and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors.

Qualifications

  • High School diploma or equivalent.
  • Graduate from an accredited LVN or RN Nursing Program, with current LVN or RN license.
  • Current CPR certification.
  • Minimum of 2 years clinical experience, Case Management experience preferred.
  • Excellent written and verbal communication skills.
  • Ability to handle and prioritize tasks in order to meet all given deadlines and productivity goals.
  • Ability to responsibly handle matters of a confidential nature.
  • Ability to work in a multi-task, high-demand environment.

Requirements

Candidates must have a high school diploma and be a graduate of an accredited LVN or RN program with a current license and CPR certification. A minimum of 2 years of clinical experience is required, with case management experience preferred.
