About the role

Pay for this role is about 40% above the California median for pharmacy roles.

The pharmacist manages daily medication activities, including clinical services and drug distribution. They are responsible for compounding and dispensing medications while ensuring a safe environment for patients.

The Pharmacist is responsible for the daily organization and supervision of medication management activities in their areas, including but not limited to provision of clinical and drug distribution services and other duties as assigned. The pharmacist maintains, identifies medication safety concerns, and additional measures to provide a safe environment with consideration to the developmental changes in the patient population. Additionally, the pharmacists compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing related duties.

Responsibilities

Follows all medication related policies and procedures. Complies with all sterile compounding regulations. Ensures orders are filled and dispensed in an accurate and timely manner. Prepares medication when needed. Follow-up suggestions for dosage adjustments, drug therapy alternatives or discontinuation of medication are appropriately made. Corrects and/or clarifies any ambiguous, non-formulary, restricted drugs, incomplete or erroneous orders by direct communication with physician. Recommends appropriate substitution. Uses technology/ pharmacy software systems safely and effectively to deliver medications and services to patients. Navigates efficiently in patient charts and communicates patient medication transfers to other units when needed. Identifies methods and approaches to simplify work processes, decrease medication wastage, and increase the quality of services provided to ensures re-dispenses are appropriate. Checks messages and responds appropriately. May assist with training and coordinating technician functions. Oversees and ensures appropriate technician communication with other members of the healthcare team Assures Black Box Warnings (BBW) and REMS requirements for use are met and are appropriately monitored and documented per designated MDRH list. Reviews medication profile on a daily basis to ensure a safe and appropriate medication regimen. Monitors antimicrobial therapy duration, cultures and susceptibilities for de-escalation. Attends and participates in multidisciplinary patient care rounds, Code Blue and Code Stroke situations, and in emergency situations such as Rapid Response Team when requested Documents and communicates follow-up note on all protocols for next shift. Provides patient consultations. Documents medication education that is given to patients. Proactively serves as an information source to the team and other departments (e.g. provides nursing staff, respiratory techs, and other healthcare providers with information regarding product). Other duties as assigned.

Qualifications

Education Doctorate Degree Pharm.D. or equivalent degree from an accredited school of pharmacy in the USA. - minimum

Experience 1 year Completion of a PGY1 and/or PGY2 residency practice experience. - preferred

Licenses and Certifications Pharmacist License - Upon Hire minimum Current Pharmacist licensure in the State of California. Basic Life Support (BLS) - Upon Hire preferred BLS certification.

Requirements

A Pharm.D. or equivalent degree and a current California Pharmacist license are required. Completion of a PGY1 or PGY2 residency and BLS certification are preferred.
