PSG is a community-based social services organization dedicated to helping individuals and families through dynamic and innovative programming.

We are seeking a part-time Youth and Adult Support / Rehabilitation Specialist to join our CCS Ancillary Services program.

JOB SUMMARY: The Comprehensive Community Services (CCS) Ancillary Services program uses goal-oriented interventions to support both youth and adult consumers who are struggling in various aspects of their lives.

The Youth and Adult Support / Rehabilitation Specialist works within a multidisciplinary team to provide goal-oriented, community-based interventions for youth and adults referred through Comprehensive Community Services (CCS).

As the Youth and Adult Support / Rehabilitation Specialist, you will work with consumers to support recovery and skill development, with a focus on strengthening family dynamics through parent coaching and psychoeducation.

KEY RESPONSIBILITIES OF THE YOUTH/ADULT SUPPORT AND REHABILITATION SPECIALIST: Provide parent coaching through mentorship and behavior modification coaching to parents to enhance their parenting skills and strengthen parent-child relationships Provide rehabilitative psychoeducation through skills training and informational resources to help consumers and their families understand and manage mental health or substance use conditions.

Analyze the consumer's capabilities, strengths, and challenges to determine the services required to meet their unique needs.

Create recovery plan goals in collaboration with team members, other agency providers, and natural supports.

Exercise judgement to intervene as an advocate for the consumer in the community and as part of their recovery team.

Attend recovery team meetings as needed.

Organize and facilitate individual meetings and structured rehabilitation activities that assist consumers in developing pro-social skills and connections.

PAY RATE: $21.00-23.00 per hour LOCATION: West Allis, Wisconsin.

Local travel throughout the Milwaukee County area is required to meet with clients within the home and other community spaces, with mileage reimbursement.

SCHEDULE: Part-time 15-20 hours per week.

Work hours are flexible scheduled by assigned client availability.

Most hours will be during the Monday-Friday workweek, with occasional weekend hours based upon client need.

Requirements

Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance. A completed Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related is required. Previous experience working with youth and adults with mental illness and/or substance use disorders is required.
