About the role

The Discharge Planner coordinates case management and discharge needs for residents in behavioral health care. They develop discharge plans, secure necessary resources, and act as a liaison between the treatment team and the community.

  • We are currently seeking a Discharge Planner for Residential Services for Riveredge Hospital.

  • The Discharge Planner is responsible for the coordination of the residents’ case management and discharge needs.
  • This includes assisting the treatment team in the development of appropriate discharge plans, the facilitation of the attainment of resources to meet this plan and the education of the residents regarding the discharge process.
  • The discharge planner is a liaison between the treatment team and community.

Qualifications

Experience

  • One (1) year of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred. Bachelor Degree in psychology, social work, or related field preferred.
  • Additional
  • Requirements: Successful completion of CPR and CPI that is provided during orientation. May be required to work overtime and flexible hours.

Requirements

A bachelor's degree in psychology, social work, or a related field is preferred, along with one year of related experience in psychiatric care. Candidates must successfully complete CPR and CPI certifications during orientation.
