Schedule: 10:00am-10:00pm
Cleveland Clinic Home Care provides skilled nursing, social services and physical, occupational and speech therapy for patients returning home after a hospital stay, outpatient procedures or doctor's visit.
Our home care team continues Cleveland Clinic's treatment at home.
We provide short term intermittent acute care at home.
Our nurses work autonomously meeting patients where they live.
This requires strong assessment skills and critical thinking.
We provide flexibility to the nurse and the patient when coordinating care within a team environment.
Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. This position is eligible for student loan repayment.
00am – 10:30pm. There are no weekends, holiday or on-call requirements.
Provide direct nursing care in accordance with established policies, procedures and protocols of the agency, taking visits on the uncovered list, late admissions and other patient visits as needed.
Participate in and support quality improvement projects.
Participate in departments and committees as assigned.
Supervise starts of care, resumptions of care and routine visits as requested.
Perform accurate and complete admission assessments and reassessments within a specified time frame.
Assess patient for safety risks, abuse and neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Assess and document patients' pain using appropriate pain scale and manage patients' pain then.
Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team.
Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
Escalate change in patient condition to patient’s physician as appropriate.
