Summary of Position

The Social Worker functions as the advocate and care manager for the residents and family. Works with residents, families, and resident support people in accordance with federal, state, and local regulations to provide emotional and social support and resources to allow the residents to exist and function at their highest potential and independent level. Responsible for upholding the standards, ethics, clinical practice and values of the State of Ohio CSWFMT Board, and McGregor.

Core Responsibilities and Duties

Conducts psychosocial assessments and care conferences with residents and families.

Facilitates treatment planning, decision-making, and communication among residents, families, and healthcare teams.

Educates residents and families about illness, treatment options, and community resources.

Coordinates ancillary services like psychological and psychiatric support.

Advocates for residents' rights and ensures effective discharge planning.

Participate in facility quality improvement and staff training.

Maintains documentation compliance and manages resident concerns.

Qualifications

  • Requires a bachelor’s degree in social work or related field.
  • Preferably have 1-2 years of healthcare experience, especially in long-term care.
  • Must possess strong interpersonal, communication, and organizational skills.
