Program Statement

The Wayne County Case Manager will work with our juvenile court programs and assist with our other counseling programs. The case manager will assist with the Strive Mentoring Program, the Youth Engagement Specialist (YES) Program and are group programming that we offer to juvenile court referred youth and their families.

What You Will Do

Work closely with therapists to provide support, community linkages, and reinforce treatment interventions to youth in Wayne County.

Monitor clients' symptoms and stressors in the home, school or community. Provide crisis intervention as needed.

Provide documentation for billing in the Electronic Health Record - AVATAR. Participate in mental health assessments and treatment planning. Update treatment plans and perform discharge summaries in a timely basis as needed.

Staff member will facilitate or assist with groups and mentoring activities.

May provide in-home support to youth and families in assist with symptom stabilization and increasing interpersonal skills.

Staff member may assist with mental health, substance use and trauma screenings at the Wayne County Juvenile Court.

The candidate must be a compassionate professional capable of working with therapists to reinforce treatment interventions for youth. Experience with mental health, substance use, and trauma screenings is required for the role.
