Board-certified or board-eligible in PM&R Fellowship in Interventional Spine or Pain Management preferred CA medical license

Must be board-certified or board-eligible in Physical Medicine and Rehabilitation with a valid California medical license. A fellowship in Interventional Spine or Pain Management is preferred.
