Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Family Practice Slingerlands, a well-established primary care practice providing comprehensive family medicine services to patients of all ages in a welcoming, team-based environment.

Position overview

As an Advanced Practitioner at CapitalCare Family Practice Slingerlands, you will provide full-spectrum family medicine care alongside experienced physicians and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and strong patient relationships, with modern tools such as telemedicine and a secure patient portal to support coordinated care and patient access.

Responsibilities

  • Conduct comprehensive patient evaluations and manage acute and chronic conditions
  • Order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals as needed
  • Provide preventive care, wellness visits, and patient education across the lifespan
  • Collaborate closely with physicians, Advanced Practitioners, nurses, and support staff to ensure continuity of care
  • Maintain accurate and timely clinical documentation in accordance with organizational standards
