About the role

The Social Worker coordinates community programs and clinical services for patients within a primary care clinic and hospital settings. Key duties include discharge planning, patient advocacy, and connecting families with essential resources to improve health outcomes.

TITLE: Social Worker-- Primary Care Clinic

Department: Behavioral Health/Social Services

Reports to: Behavioral Health/Social Services Manager

Position status/Hours

  • Full-time .9 FTE (36 hours/week). Monday through Friday- with hours scheduled 5 days per week.
  • Scheduled hours will include starting at 8a and ending at 3:30p with the exception of one day a week ending at 4p.

Job summary

The Social Worker works with patients to coordinate community programs and clinical service needs. The Social Worker works closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.

The social worker will spend the majority of time in the Primary Care Clinic in Lancaster, with flexibility to travel to outreach clinics when needed (Fennimore, Cassville and Potosi). The social worker will also provide regular social work coverage on Med/Surg for social work needs and discharge planning. The social worker will be available to assist in the Emergency Department, Specialty Clinics, Outpatient/Rehabilitation Departments, OB, and other areas of the hospital to provide social work assistance and discharge planning upon request.

Social worker job duties: o Maintain a positive, respectful and professional attitude;

o Demonstrate professional verbal and written communication skills effectively;

o Proactively act as an advocate for patients and families and utilizes a patient centered approach;

o Demonstrates awareness, understanding and respect of cultural diversity and patients of all ages and socioeconomic groups;

o Demonstrates the ability to be flexible, organized and function under stressful situations;

o Proficiently and accurately document all patient interactions in medical record and ensure all documentation meets current standards and policies;

o Continuously expand knowledge and understanding of resources and services available within the community;

o Develop and maintain relationships with community organizations such as the ADRC, social services, long term care facilities, home health agencies, etc.;

o Collaborates and supports Social Services team by providing coverage for time off, assisting with addressing patient needs in areas such as ER, OB, outpatient departments and provides regular/weekly social work coverage on Med Surg for discharge planning needs;

o Assist with care planning and transitions with linkage to resources and facilitation of referrals to optimize wellbeing;

o Assess patients and families to identify needs (housing, food, mental health, transportation, financial concerns, etc.) and connect them to resources to prevent illness, reduce barriers to care and enhance overall stability and well-being;

o Assist with No Call/No show patients and assist them in reducing barriers to attending their appointments;

o Ability to work independently and collaboratively with interdisciplinary care teams, medical providers and other agencies;

o Provide support to Chronic Care Management program;

o Assist OB patients access resources during pregnancy and after birth;

o Arrange for medical equipment, home services and transportation as needed;

o Provide advance care planning and assist with advance directives for patients and community members;

o Serve as a resource for identifying and reporting child, elder and dependent adult suspected abuse or neglect;

o Participate in relevant community events and programs;

o Support and participate in swing bed program and ethics committee.

Qualifications

1. Bachelor’s Degree in social work or related field with a Wisconsin Certified Social Worker (CSW) license, in good standing, required.

2. Minimum of 1-year social work experience in a health care setting, clinic setting or casement experience.

3. Experience with EPIC medical record system is beneficial but not required.
