What You'll Do Provide skilled, age-appropriate nursing care to hospice patients in homes and residential care facilities in accordance with New Hampshire Board of Nursing standards and Hospice best practices.

Educate patients, families, and caregivers about the Hospice philosophy, available services, care expectations, and bereavement support.

Develop, implement, and update individualized plans of care in collaboration with physicians, patients, caregivers, and the interdisciplinary hospice team.

Perform comprehensive patient assessments and ongoing reassessments, documenting care accurately and thoroughly.

Deliver treatments and nursing interventions while monitoring patient progress and adjusting care plans as needed.

Empower patients and caregivers through education that promotes comfort, safety, and confidence in care.

Collaborate closely with physicians, nurses, social workers, chaplains, volunteers, and other healthcare professionals to ensure coordinated, high-quality care.

Participate in interdisciplinary team meetings, patient care conferences, quality improvement initiatives, and agency committees.

Attend agency in-service trainings and continuing education opportunities to support professional growth.

Requirements

Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC). Three to four years of recent general medical/surgical or skilled/acute care nursing experience. Valid driver's license, reliable transportation and an acceptable driving record.
