Provide skilled nursing assessments and interventions in patients' homes. Develop, implement, and evaluate individualized care plans. Administer medications, treatments, and other physician-ordered services. Monitor patient progress and communicate changes in condition to physicians and other healthcare providers. Educate patients and families regarding disease management, medications, treatment plans, and health promotion. Complete accurate and timely clinical documentation in accordance with agency, state, and federal requirements. Participate in public health programs and initiatives as assigned. Coordinate care with interdisciplinary team members and community resources. Maintain patient confidentiality and comply with HIPAA regulations. Attend required meetings, trainings, and continuing education activities.
Required Current Iowa Registered Nurse (RN) license in good standing. Valid driver's license. Reliable personal vehicle for travel throughout Franklin County. Current automobile insurance meeting state requirements. Excellent communication, organizational, and critical thinking skills. Ability to work independently and manage a flexible schedule. Preferred Home health, public health, community health, or case management experience. Experience working with diverse patient populations.
