A great physical therapist is one who develops a realistic plan with measurable goals tailored to each patient’s situation, with consideration to the physician, family, and other healthcare providers. A great physical therapist doesn’t just go through the motions, instead listening carefully to a patient’s concerns, monitoring progress, and adjusting plans as needed. At Franciscan Health, we are looking for a great physical therapist who demonstrates these important qualities.

What you can expect

SIGN ON BONUS AVAILABLE- (Ask your Talent Acquisition Advisor if you are eligible for Sign-On Bonus)

Complete appropriate goal re-assessment and discharge planning.

Perform comprehensive physical therapy evaluations, treatment, and related follow-up services, including discharge planning.

Document patient assessments and pertinent information in a timely manner.

Provide services and treatments requiring physical therapy skills, according to the written plan of care and the signed plan of treatment.

Supervise Physical Therapist Assistants and Home Health Aides.

Train and educate patients and family members or others support, and document understanding of treatment plan and goals.

Qualifications

  • Bachelor's Degree- Physical Therapy - Required
  • 1 Year Physical Medicine/Rehabilitation Experience- Required
  • 1 Year Home Health Experience - Preferred
  • Physical Therapist (PT) State Licensing Board - Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
  • Driver's License - Required
  • Auto Insurance - Required

Travel is required: Up to 75%

Requires a Bachelor's degree in Physical Therapy, state licensure, and at least one year of experience in physical medicine or rehabilitation. Must possess a valid driver's license, auto insurance, and CPR certification.
