Overview Join a mission-driven hospice team serving the Keene, Peterborough, and Charlestown, NH communities. As a Care Transitions Specialist, you'll build relationships with hospitals and healthcare facilities, support patients and families during critical transitions, and help ensure timely access to hospice care. Bachelor’s degree in Nursing or Social Work At least 3 years of experience, preferably in home health, hospice, acute care, hospital, or community health settings.
