About the role

Build and maintain relationships with healthcare facilities to identify patients who would benefit from hospice services. Coordinate care plans and support smooth transitions for patients and families moving into hospice care.

Overview Join a mission-driven hospice team serving the Keene, Peterborough, and Charlestown, NH communities. As a Care Transitions Specialist, you'll build relationships with hospitals and healthcare facilities, support patients and families during critical transitions, and help ensure timely access to hospice care. Requirements

Bachelor’s degree in Nursing or Social Work At least 3 years of experience, preferably in home health, hospice, acute care, hospital, or community health settings.
