Goodhue County Health & Human Services is seeking qualified applicants for a Care Coordinator

Provide case management, care coordination and comprehensive assessments to Goodhue County residents.

Examples of Work

Complete MNChoices or assessments in home and community settings.

Assess and evaluate client’s health and living situations to determine level of care and services needed.

Develop person-centered care plans to address service needs in the community

Collaborate with community providers to meet client needs

Provides input in program planning, the development of agency goals and the evaluation of the agency’s programs & policies.

Manage individual budgets for approved services

Maintains case file documentation.

Work as part of the Home and Community based services and Health plan team.

Minimum Qualifications

  • Bachelor degree in social work or related field with one (1) year experience in human services or highly related home and community-based service field or
  • Associate or Bachelor degree by the National League of Nursing and licensed as a Registered Nurse in Minnesota with one-year experience in public health nursing or home and community-based service field. or
  • A Master’s degree in nursing, social work or related field with one year of experience in home and community-based service field.
  • Must have a valid driver’s license.
  • Ability to complete and pass MNCHOICES certification upon hire.

Compensation/Benefits

County employees receive competitive pay and a generous benefit offering. The Care Coordinator is compensated at salary grade 112 ($73,860.80 - $114,524.80) and starting annual compensation is $73,860.80.

The County’s comprehensive benefit package includes

medical, dental, vision and accident insurance; health savings account; employer paid life insurance; short and long-term disability insurance; pre-tax flexible spending accounts; retirement plan; tax-deferred compensation; vacation and sick leave; and 13 paid holidays per year. Requirements
