Goodhue County Health & Human Services is seeking qualified applicants for a Care Coordinator
Provide case management, care coordination and comprehensive assessments to Goodhue County residents.
Complete MNChoices or assessments in home and community settings.
Assess and evaluate client’s health and living situations to determine level of care and services needed.
Develop person-centered care plans to address service needs in the community
Collaborate with community providers to meet client needs
Provides input in program planning, the development of agency goals and the evaluation of the agency’s programs & policies.
Manage individual budgets for approved services
Maintains case file documentation.
Work as part of the Home and Community based services and Health plan team.

County employees receive competitive pay and a generous benefit offering. The Care Coordinator is compensated at salary grade 112 ($73,860.80 - $114,524.80) and starting annual compensation is $73,860.80.

Requires a Bachelor's degree in nursing (with RN licensure) or a degree in social work, psychology, sociology, or a related human services field. Must possess a valid driver's license and be able to complete MNCHOICES certification.
