Overview

  • This role provides support and helps identify resources for clients recovering from psychiatric illnesses under clinical supervision, assisting with their treatment. It may also involve offering substance abuse services, education, support, and guidance to families. The role includes providing crisis intervention as necessary and playing a key part in the crisis counseling, stabilization, and treatment team. Additionally, the position is responsible for coordinating care for BNet clients, including overseeing the payment of services and goods, meeting with families monthly, and working closely with the BNet Liaison to ensure these tasks are completed efficiently.

  • RESPONSIBILITIES: Completes necessary assessments such as but not limited to the Bio-Psychosocial Preliminary Assessment, Medical Questionnaire, and the Medication/Verification form in line with the Client Record Handbook. Based on the assessment, develops a service plan in conjunction with the client and significant others, outlining the necessary steps to address the client’s needs. Schedules routine home/community visits with clients and their families to monitor progress. Participates in quarterly team meetings to discuss the treatment status and recommendations, ensuring that the client’s progress and needs are continually evaluated and addressed. Provides advocacy and wraparound services to meet each client’s social, educational, nutritional, and physical needs. Meets the Service Productivity standard of at least 75% (or 6 out of 8 work hours) for the provision of Case Management services in accordance with applicable clinical/quality criteria. Supports this by ensuring appropriate documentation that meets the Client Record Handbook specifications and Agency business/clinical ethical standards. Assists clients in gaining access to financial and insurance benefits, employment, medical, social, educational, and other services. Provides information and referrals to assist clients with needs such as medical, dental, occupational, residential, and social services, as well as other barriers to stabilization. Works collaboratively with the client and their natural support system to address and meet the client’s needs. Coordinates the client’s care with both external and internal service providers, including the Crisis Stabilization Unit (CSU) and other necessary services. Initiates referrals related to clinical recommendations for treatment and assesses the need for additional services and support. Tracks service units and the frequency of services provided to clients, working with service providers to maintain compliance with the service agreement. Documents all services provided in the client record in accordance with Client Record Handbook Guidelines. Progress notes must clearly reflect how the support coordinator’s efforts are linked to the services and the client’s progress. Ensures at least 85% compliance with progress note requirements during Clinical Reviews and maintains program case files in accordance with Center standards, Agency policy, F&F rules, and JCAHO standards.
  • QualificationsEducation/Experience: Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, health science, or a related human services field preferred.

  • Certification: Current valid driver’s license. Maintain Apalachee Designated Auto Driver status.
