Canyon Utah Home Care & Hospice - Ogden - South Ogden, UT 84403A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager. QUALIFICATIONS Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. One (1) year nursing experience, preferred. Experienced Home health nurse preferred. Acceptance of philosophy and goals of this Agency. Ability to exercise initiative and independent judgment. QualificationsHome Health Float Nurse – Responsibilities The Home Health Float Nurse provides skilled nursing care while supporting clinical operations across the agency. This role offers flexible coverage to ensure continuity of patient care, assists with staff development, and provides additional clinical support wherever needed. Float Role Responsibilities Provide coverage for nurses during PTO, leave of absence, illness, and other staffing needs. Complete overflow admissions, recertifications, resumption of care visits, routine visits, and discharges as assigned. Assist clinicians with high caseloads or complex patient situations to maintain quality patient care. Provide dual visits and mentorship for newly onboarded nurses and ongoing staff education. Serve as a clinical resource to support best practices, documentation, and patient care. Promote continuity of care by collaborating with case managers and the interdisciplinary team during coverage assignments. Demonstrate flexibility by adapting to changing schedules, patient assignments, and agency priorities. Clinical Responsibilities Provide skilled nursing services in accordance with the physician's orders and individualized plan of care. Complete comprehensive assessments, determine homebound status and eligibility, identify immediate patient needs, and develop or revise the plan of care in collaboration with the interdisciplinary team. Perform OASIS assessments, reassessments, transfers, resumptions of care, recertifications, significant change assessments, and discharges within regulatory and agency timeframes. Regularly reassess patient condition and communicate changes to physicians and appropriate team members. Provide skilled nursing interventions, preventive care, rehabilitation, patient and caregiver education, and disease management. Process physician orders, obtain verbal orders as needed, and complete certifications, recertifications, and discharge documentation. Utilize appropriate ICD-10 diagnosis coding and maintain accurate clinical documentation. Care Coordination Collaborate with physicians, therapists, social workers, home health aides, patients, caregivers, and other healthcare providers to ensure coordinated care. Refer patients to additional disciplines when specialized services are indicated. Conduct and participate in interdisciplinary care conferences. Assign and supervise Home Health Aides and perform required supervisory visits in accordance with regulatory requirements. Supervise LPN/LVNs according to agency and regulatory requirements. Documentation & Quality Complete all clinical documentation accurately and within agency timeframes. Maintain timely progress notes, care conference summaries, and required regulatory documentation. Participate in quality improvement initiatives, utilization review, peer review, and performance improvement activities. Immediately report patient incidents, complaints, safety concerns, or significant variances to leadership. Professional Responsibilities Maintain current CPR certification and all required licensure. Attend staff meetings, educational programs, and required in-service training. Demonstrate professionalism, sound clinical judgment, critical thinking, and effective time management. Maintain patient confidentiality and comply with all HIPAA, Medicare, state, and agency policies. Maintain dependable attendance, professional appearance, and a clean work environment. Participate in on-call rotation as assigned. Perform other duties as assigned.

Requirements

Candidates must be a graduate of an approved professional nursing school and hold a current state license. One year of nursing experience is preferred, with a strong preference for those experienced in home health.
