The LPN/LVN provides quality nursing care to hospice patients in their homes under the direction of a registered nurse. Responsibilities include collecting assessment data and implementing care plans within the scope of licensure.
This position will be traveling to see hospice patients in the Columbia Gorge service area.
Schedule: Weekdays day shift with rotating weekend coverage
The LPN/LVN works under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assists with collection of assessment data and provides input to the planning of care. Implements plan of care within the scope of licensure. Performs all duties in a manner which promotes team concept and reflects the Providence mission and philosophy.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Candidates must be graduates of an accredited LPN/LVN school with active nursing licenses in both Oregon and Washington. A minimum of one year of nursing experience and a valid driver's license with auto insurance are required.
