Administers physician-prescribed physical therapy to patients in their residences to improve physical disabilities and promote wellness. Responsible for initial assessments, developing plans of care, and maintaining detailed clinical records including OASIS assessments.
Minimum Requirements: Must be a licensed Physical Therapist in the State of Montana preferably with one to two years related experience. Must have a valid driver’s license and vehicle insurance. PHYSICAL DEMANDS: ➢ Ability to work for extended period of time while standing and being involved in physical activity. ➢ Heavy lifting. ➢ Ability to do extensive bending, lifting, and standing on a regular basis. WORKING CONDITIONS: ➢ Works in patient homes, independent living communities, and assisted living communities. ➢ Work involves some exposure to hazards, blood borne pathogens, or physical risks, which require following basic safety precautions. ➢ Work involves exposure to plants and animals within the patients’ home environments. ➢ Travel to/from patient home environments.
