We are seeking a compassionate and dedicated Licensed Social Worker to join our team. The ideal candidate will have a strong background in geriatrics and hospice care, with experience in an outpatient setting. This role involves delivering a wide range of social work services to hospice patients and their families, including: Conducting initial emotional, spiritual, and psychosocial assessments Providing ongoing counseling and support Offering bereavement services Participating in community education, outreach, and referrals As a key member of the Interdisciplinary Group (IDG), the Hospice Social Worker plays an integral role in assessing patient needs, developing individualized care plans, and ensuring compliance with hospice care standards. The position will cover our Ridgeland and Vicksburg, MS office location area, including the surrounding and outlying communities.
Must be a Licensed Social Worker with a strong background in geriatrics and hospice care. Experience in an outpatient setting is preferred.
