Job Description

I. Purpose and Overall Duties The Physical Therapist (PT) evaluates, plans, and implements individualized physical therapy programs for patients in a long-term care or related setting. The role focuses on improving mobility, strength, balance, and functional independence while reducing pain and preventing further injury. II. Essential Job Functions A. Patient Evaluation and Treatment Planning Evaluate patients’ physical condition, mobility, strength, balance, and functional abilities using standardized assessments and clinical judgment Develop individualized treatment plans based on patient needs, physician orders, and regulatory requirements Establish measurable goals and modify treatment plans as necessary based on patient progress B. Patient Care / Intervention Implement therapeutic interventions to improve mobility, strength, balance, coordination, and endurance Provide gait training, transfer training, and therapeutic exercise programs Utilize manual therapy techniques and therapeutic modalities as appropriate Educate patients and caregivers on exercises, safety precautions, and home programs Recommend and train patients in the use of assistive devices (e.g., walkers, canes) C. Documentation / Compliance / Outcomes Complete all required clinical documentation accurately and timely in accordance with Medicare, regulatory, and company guidelines Maintain appropriate records of patient evaluations, progress notes, and discharge summaries Ensure compliance with all federal, state, and payer regulations Monitor patient outcomes and adjust interventions to support optimal results D. Collaboration / Supervision / Leadership Collaborate with interdisciplinary team members to coordinate patient care Supervise and direct Physical Therapist Assistants (PTAs) and support staff as applicable Communicate effectively with physicians, nursing staff, and families regarding patient progress Participate in care planning meetings and team conferences E. Professional Development Maintain current licensure and clinical competencies Participate in continuing education and professional development activities Support evidence-based practice and quality improvement initiatives III. General Skills Required Strong clinical reasoning and decision-making skills Effective communication (verbal and written) Interpersonal and relationship-building skills Time management and organizational skills Problem-solving and adaptability Ability to work collaboratively within an interdisciplinary team IV. Minimum Experience, Education, and Licensure Doctor of Physical Therapy (DPT) or equivalent degree Current state licensure as a Physical Therapist One (1) year of experience preferred V. Essential Physical Demands Ability to assist patients with mobility, transfers, and ambulation using appropriate techniques and assistive devices Ability to stand, walk, and move throughout the workday Frequent physical interaction with patients, including supporting or guiding movement during therapy Occasional bending, reaching, or positioning of patients during treatment Ability to use proper body mechanics in accordance with safety standards VI. Work Environment Long-term care or clinical setting Ability to travel to various locations where patients are located, including long-term care, short-term care, and private homes. This work will involve ability to enter and access buildings, work with patient in their existing space. Varying environmental conditions may be encountered while traveling. Indoor environment with occasional travel between patient areas or facilities as needed VII.
