Physical therapist, part-Time
Hopedale Medical Complex is seeking a compassionate and motivated Physical Therapist to join our Rehabilitation Services team. In this role, you will evaluate, develop, and implement individualized treatment plans that help patients restore function, improve mobility, reduce pain, and maximize independence.
As a valued member of our multidisciplinary rehabilitation team, you will collaborate with physicians, nursing staff, occupational and speech therapists, case management, and rehabilitation support staff to deliver exceptional, patient-centered care. Essential responsibilities
Evaluate patients and develop individualized treatment plans.
Provide skilled physical therapy treatments using evidence-based techniques.
Monitor patient progress and modify treatment plans as appropriate.
Educate patients and caregivers on home exercise programs, mobility, safety, and use of assistive devices.
Collaborate with physicians and interdisciplinary healthcare teams to optimize patient outcomes.
Complete accurate and timely clinical documentation.
Participate in discharge planning and continuity of care.
Mentor Physical Therapist Assistants, students, and rehabilitation support staff as appropriate.
Participate in quality improvement initiatives and continuing education.
Maintain compliance with HIPAA, accreditation standards, and HMC policies.
