The coordinator is responsible for initial and ongoing assessments and creating individualized recovery plans for an SMI population. They provide person-centered interventions for individuals, groups, and families while collaborating with other providers for discharge planning.

Education Requirements: Master's degree required; Professional licensure required;
Valid driver's license in good standing preferred
Proficient in Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint) and database navigation required.
