Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area. Conduct comprehensive assessments of patients' social and emotional needs Assist with coordination of care and access to community resources Maintain detailed and accurate documentation of patient interactions and progress Utilize Homecare Homebase EMR (knowledge of which is a plus) Develop personalized care plans in collaboration with the interdisciplinary team Provide counseling and support to patients and families facing end-of-life challenges
Licensed as an LMSW Knowledge or experience utilizing Homecare Homebase EMR would be a plus Minimum of one year of hospice care experience Minimum of one year of social work experience Compassionate and empathetic approach to working with the elderly Strong clinical and assessment skills Excellent communication and interpersonal abilities Ability to work independently and manage time effectively Payrate: $55-$75 per visit
