Trinity Health PACE Alexandria is a Program of All-Inclusive Care for the Elderly (PACE) in Central Louisiana that provides comprehensive medical, social, and supportive services that enable older adults to remain safely in their homes and communities. Serving eligible seniors throughout Rapides and Avoyelles Parishes, PACE integrates healthcare, rehabilitation, transportation, home care, and caregiver support through an interdisciplinary team dedicated to delivering person-centered care.

Position Purpose

The Admission Nurse is responsible for completing the initial and ongoing comprehensive participant assessments utilizing the interRAI Home Care (iHC) assessment tool for prospective and enrolled PACE participants. The RN serves as a key member of the team, gathering clinical, functional, cognitive, psychosocial, and environmental data to determine participant needs, support care planning, and ensure compliance with PACE, CMS, and Louisiana Medicaid requirements. The iHC assessment is specifically required as part of the PACE eligibility and care planning process.

Duties and Responsibilities

  • Assessment and Enrollment
  • Complete comprehensive pre-enrollment assessments using the interRAI Home Care (iHC) assessment tool.
  • Conduct face-to-face assessments in the participant's home, PACE center, hospital, nursing facility, or other care setting as appropriate.
  • Evaluate:
  • Medical status
  • Functional abilities (ADLs/IADLs)
  • Cognitive status
  • Behavioral health needs
  • Medication management
  • Nutritional status
  • Caregiver support systems
  • Home safety and environmental risks
  • Identify unmet needs, risk factors, and opportunities to support participants safely in the community.
  • Present assessment findings to the IDT to support enrollment decisions and care plan development.
  • Work as a liaison between Sales and Enrollment, Medical Director, outside, the Interdisciplinary Team, and outside medical professionals and/or vendors to ensure a safe transition of care onto the PACE program.
  • Ongoing Assessment and Reassessment
  • Complete scheduled reassessments and significant change assessments utilizing the interRAI Home Care instrument.
  • Monitor changes in condition and communicate findings to the IDT.
  • Recommend modifications to participant care plans based on assessment outcomes.
  • Ensure assessment data accurately reflects current participant status and service needs.
  • In the absence of initial or ongoing assessments, assist the Clinic Manager with assigned duties, as needed

Minimum Qualifications

  • Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse in the state of PACE Organization. BSN preferred.
  • MDS experience preferred..
  • Minimum of two (2) years of documented experience working with a frail or elderly population or if less than two (2) years of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Valid driver's license in good standing and proof of insurance required.
  • Must have the ability to perform standard clinical procedures according to established protocols and possess initiative, solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services.
  • Experience with interRAI Home Care (iHC) assessments is preferred but not required. Trinity Health PACE Alexandria will provide training and mentorship to the selected candidate to develop proficiency in the iHC assessment process and its role in participant care planning and enrollment.

Requirements

Candidates must be a Registered Nurse with a valid license and preferably a BSN, with at least two years of experience working with frail or elderly populations. A valid driver's license and proof of insurance are required for home visits.
