Under clinical practice guidance of the Social Work Supervisor, the Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.

Job Description

Location: Lexington County Detention Center

Day Shift / 9

00am - 5:30pm

Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Education

Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.

Credentials

MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
