  • Category: Administrative - ClericalESSENTIAL DUTIES AND RESPONSIBILITIES The Senior Center Social Worker serves as a vital resource for older adults and their families within our older adult center. This role involves conducting psychosocial assessments, developing individualized care plans, and connecting participants with essential government benefits like Medicare and Medicaid. The Social Worker acts as a liaison and advocates, facilitating support groups and organizing wellness programs to combat social isolation. This position requires a compassionate professional capable of crisis intervention and collaborating with interdisciplinary teams to foster a supportive environment for diverse aging populations.
  • Specific responsibilities and duties include:
  • Advocate for clients to resolve challenges
  • Collaborate with other professionals to evaluate client’s medical or physical conditions and assess client’s needs.
  • Refer client or family to community resources to assist in recovery from mental and physical illness and to provide access to services such as financial assistance, legal aide, housing, education, Benefits, Entitlement and job placement.
  • Counsel clients in individual and group sessions to help them improve quality of life.
  • Organize support groups on topics such as anxiety, bereavement, depression, etc.
  • Organize support groups or counsel for family members to assist in understanding and supporting clients.
  • Maintain written records of contact with clients, including case files, worker logs, and computerized client databases.
  • Attend training as required, including mandatory funding organizations.
  • Assist agency in outreach activities in community.
  • Abide by all HIPAA policies and procedures
  • Other duties as required by the Director. COMPETENCIES
  • Demonstrated experience developing and delivering comprehensive assistance to clients
  • Experience conducting needs assessments and using results to guide programmatic decisions.
  • Articulated knowledge of health, mental health and wellness issues
  • Demonstrated application of evidence-based and evidence-informed health practices.
  • Demonstrated understanding of and stated commitment to diversity, inclusion, and equity issues.
  • Demonstrated experience working with diverse populations.
  • Demonstrated ability to facilitate presentations to large and small audiences.
  • Demonstrated ability to communicate information effectively.
  • Demonstrated interpersonal skills necessary for effective work relationships.
  • Bilingual (Spanish/Cantonese/Mandarin) required. EDUCATION and EXPERIENCE
  • Master’s degree in Social Work.
  • At least two years’ experience in providing social services required.
  • Experience in providing case management or social services within the senior population required. Requirements

A Master's degree in Social Work and at least two years of experience in social services are required. Candidates must have specific experience with the senior population and be bilingual in Spanish, Cantonese, or Mandarin.
