Develop and implement individualized physical therapy treatment plans for clients within their home residences. Collaborate with physicians and multidisciplinary teams to establish patient-centered goals and revise care plans as needed.

This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client’s home/residence.
Essential functions
Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting.
Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.
Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.
Communicates with physician any requests to revise the plan of care.
Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.

Must be a graduate of an APTA-approved university and hold a valid physical therapy license in the state of Iowa. A minimum of one year of experience is preferred, with community or home health experience being desirable.
