About the role

Provide psychosocial rehabilitative services to children and families to help them meet individualized treatment goals. Responsible for maintaining regulatory compliance, completing documentation, and utilizing evidence-based practices.

Description Empower Children and Families Valley's Children, Youth, & Family Program provides a safe, structured, and therapeutic environment where young people can work through mental and behavioral health challenges that impact their success at home, school, and in the community. As a Case Manager working toward certification, you'll gain valuable hands-on experience providing rehabilitative services, supporting individualized treatment goals, and helping children and families navigate the path toward healing and growth.

Pay

Dependent on Case Manager certification status (see below) Uncertified start at $17.50 - will obtain certification within the first 120 days and pay will be increased Certified range starts at $18.75 - pay is calculated based on years of certified experience.

Schedule: Monday-Friday | 8:00am-5:00pm | Hours may vary

Health Savings & Reimbursement Out-of-pocket medical expenses may be eligible for reimbursement through our Garner HRA — up to $2,000 for individuals and $4,000 for families. Depending on your medical plan selection, you may also contribute pre-tax dollars to a Health Savings Account (HSA), with a company match of up to $900 for individuals and $1,800 for families. Retirement Our 401(k) plan accepts both pre-tax and Roth (post-tax) contributions and includes a company match of up to 6% of your annual salary. Job Summary & Deliverables This title is used for Case Managers who are working toward certification. Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met. Provides psychosocial rehabilitative services as described by the Medicaid manual Supports clients in following individualized care plans and identifying case management goals Completes documentation on services provided within Valley’s standards Meets productivity expectations as assigned by supervisor Distributes medications as necessary Transports clients to appointments as necessary

Requirements

Education High School diploma or equivalent Licenses/Certifications Case Manager certification must be obtained within 120 days Enrolled in ValleyAcademy for Case Managers (post hire) CPR certification (post hire if needed) Valley de-escalation certification (post hire) Driving positions require a current Driver’s License, must be 21

Preferred Qualifications: Previous behavioral health experience Bachelor’s degree in social services, Psychology, or related field
