About the role

Provide care coordination and support to pregnant and postpartum women struggling with substance use disorders. Collaborate with medical providers and community resources to facilitate therapy, medical referrals, and assistance with housing and food resources.

Job Summary

The Care Coordinator

Works within the Soft Landing Program at the Reading Hospital. Possess thorough knowledge of substance use disorder, case management theory, motivational interviewing, child development and medication assisted treatment as well other available treatment options and maternal health needs. Provides care coordination services to assigned program participants. Builds supportive appropriate relationships with program participants. Works collaboratively with medical providers and community resources. Sets clear healthy limits and professional boundaries between coordinator and patient. Commutes to patients as needed and works independently to meet patient needs. Takes part in ongoing data collection and quality improvement measures. What is the Soft Landing Program? The Soft Landing Program at Reading Hospital provides support to pregnant and postpartum women struggling with substance use. The program serves those who are currently using substances, those in recovery, and individuals that are utilizing Medication-Assisted Treatment. The program provides patients with care coordination, therapy, medical care referrals, recovery and mental health resources, pregnancy and parenting support, and assistance applying for resources such as food assistance and housing. #READ #LI-AH1

Qualifications
