Works within the Soft Landing Program at the Reading Hospital. Possess thorough knowledge of substance use disorder, case management theory, motivational interviewing, child development and medication assisted treatment as well other available treatment options and maternal health needs. Provides care coordination services to assigned program participants. Builds supportive appropriate relationships with program participants. Works collaboratively with medical providers and community resources. Sets clear healthy limits and professional boundaries between coordinator and patient. Commutes to patients as needed and works independently to meet patient needs. Takes part in ongoing data collection and quality improvement measures. What is the Soft Landing Program?
