How you'll contribute
Identifies the targeted population and implements clinical interventions and protocols.
Collaborates with PCP, patients and members of health care team including continuum of care settings and community resources.
Provides self-management support and includes patient/family as active members in the team approach.
Facilitate close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.
Coordinates care between PCP and acute care hospitals, specialists and post
acute care services.

What we're looking for
Applicants should have a current Michigan RN license and possess an associate’s degree from an accredited nursing school (bachelor’s preferred).

Applicants must hold a current Michigan RN license and an associate's degree in nursing, though a bachelor's degree is preferred. Required qualifications include BLS-HCP certification and 2 years of experience in adult and pediatric medicine.
