Full-Time Outpatient Palliative Care Nurse Practitioner (NP) Atlantic County, New Jersey Are you a compassionate and highly skilled Nurse Practitioner passionate about providing meaningful, patient-centered care to individuals with serious illness?

We are seeking an experienced Palliative Care NP to join our growing team and make a profound impact in the lives of patients and families throughout Atlantic County, NJ.

Position Highlights Full-Time Salaried Position Established patient panel averaging 120+/- patients Meaningful, relationship-driven patient care Collaborative interdisciplinary support Opportunity to guide patients and families through complex care journeys What You’ll Do As an Outpatient Palliative Care Nurse Practitioner, you will play a critical role in improving quality of life for patients facing serious or complex illness by providing compassionate symptom management, advance care planning, and care coordination.

Key Responsibilities Assess and manage pain and other complex symptoms in the outpatient setting Lead compassionate goals-of-care and advance care planning conversations Support patients and families with hospice readiness and facilitate hospice referrals when appropriate Collaborate closely with physicians, specialists, home health, hospice teams, and community referral partners Participate in interdisciplinary team meetings and care conferences Coordinate seamless transitions across outpatient, hospital, home health, and hospice settings Serve as a trusted clinical resource and advocate for patients and families

Qualifications
