Clarvida is seeking a Care Coordinator to provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services to children and adults with behavioral or mental health diagnoses. In this role, you will meet with clients and families in their homes to assess strengths and emotional, behavioral, and physical needs, develop individualized treatment plans, and connect clients with appropriate community-based resources. You will engage clients and families regularly to support progress toward goals and collaborate with educational, vocational, medical, and behavioral health providers to deliver coordinated, wraparound services.
