Eastern Nephrology is seeking a dedicated and skilled Registered Nurse to join our clinical team in North Carolina. The Registered Nurse will provide high-quality, patient-centered care, working closely with medical providers to execute individualized care plans. This role involves direct patient care for adult and geriatric populations, along with documentation and clinical support activities in line with our established protocols and standards.

Patient Care: Develop and implement nursing care plans and provide professional nursing care to adult and geriatric patients. 50% 1. Perform Chronic Care Management activities and assist with patient intake. 2. Obtain and record patient vitals, including blood pressure, temperature, pulse, and weight. 3. Review and document patient allergies, medications, and any over-the-counter drugs in the EMR system.
Clinical Documentation: Accurately document patient interactions, changes in care interventions, and communication with external providers in the EMR. 40% 1. Maintain medical records with updates on triage, referrals, and prescription refill requests.
Supportive Care: Assist with patient inquiries, hospital calls, lab and test results, and prescription refills as needed. 10%

Work Schedule is Monday- Friday, 8 am to 5 pm with a one-hour lunch. Slight modifications may be made to this schedule with approval from manager.
