  • Summary: As the Housing Case Manager you will provide case management to survivors of Domestic Violence experiencing housing instability. The role includes conducting assessments, developing housing stability plans, and connecting participants to community resources that support long-term housing stability and self-sufficiency. This position is responsible for maintaining accurate case documentation, managing HMIS and reporting requirements, monitoring participant progress, ensuring program compliance, and collaborating with community partners to support successful program outcomes.

  • Duties/Responsibilities: Prospects and develops new landlord relationships to provide appropriate housing stabilization for individuals experiencing homelessness. This includes intensive advocacy focused on permanent housing stability, housing relocation services, and financial assistance. Provides individual case management services to participants, including weekly one-on-one meetings, regular phone check-ins, and/or Zoom meetings. Codevelops action plans with participants that address their crisis without creating unsustainable tradeoffs, while maximizing participant wellbeing. These plans outline goals related to education, employment, childcare, and maintaining housing. Assists participants in obtaining appropriate substance abuse, mental health, and/or medical services as needed. Coordinates immediate and longer-term access to resources such as transportation, flex funding, and permanent housing, in conjunction with collaborating agencies. Initiates and maintains collaborative partnerships with domestic and sexual violence service providers, other social service professionals, businesses, and informal resources in the communities where participants reside. Answers phone calls, conducts intakes of new participants, and provides referrals and information to potential housing participants. Completes data entry, maintains HMIS records, and submits reports required for grant compliance. Attend tabling events as needed, this may consist of some weekend flexibility when needed. Stays current on Central Massachusetts housing issues by building and maintaining relationships with housing and social service agencies in the region. Prepares and delivers community presentations in Worcester on domestic violence and housing when requested. Assists and collaborates with team members as necessary. Performs other duties as requested by the Housing Manager or YWCA management team. Attend all mandatory Domestic Violence meetings and trainings along with housing trainings.
  • QualificationsRequired
  • Skills/Abilities: Familiarity with domestic violence and/or gender issues. Strong organizational skills and attention to detail. Bilingual proficiency in English and Spanish preferred. Computer literacy (Word, Excel, and general internet skills). Experience working with individuals from different cultures and backgrounds. Experience developing and delivering community presentations. Demonstrated familiarity with LGBTQ+, HIV/AIDS, mental health, and/or substance abuse populations. Access to a vehicle to facilitate travel between the agency's areas of coverage. Comfort working both individually and in teams. Innovative thinking in approaching issues of poverty and service delivery. Familiarity with social services in Greater Worcester and North Central Massachusetts. Ability to work a flexible schedule, including possible adjustments to weekday hours or one weekend day to accommodate participant needs.

  • Experience: A minimum of two years' experience in human or social services. Bachelor's degree preferred, or an equivalent combination of education, training, and experience.

Requirements

Requires a minimum of two years of experience in human or social services and a bachelor's degree is preferred. Candidates should have familiarity with domestic violence issues and access to a vehicle for travel.
