  • Schedule: PRN day shift, Mondays through Fridays only What You Will Do Lead the execution of personalized care plans, ensuring every clinical intervention is tailored to meet the unique physical and emotional needs of your patients. Monitor and communicate patient milestones with precision, acting as the essential link between real-time bedside observations and the broader medical strategy. Coordinate the multidisciplinary care team to ensure seamless collaboration across all specialties, providing a unified and holistic healing experience for the patient. Execute physician and nursing orders with clinical excellence, maintaining the highest standards of safety and accuracy in every treatment provided. Champion the discharge planning process by collaborating with the healthcare team to ensure patients and their families have a clear, confident, and safe transition back to their homes.

Requirements
