The Integrated Health Clinic seeks a compassionate and experienced Advanced Practice Provider (APP) to provide comprehensive outpatient primary care within an integrated practice delivering primary care, psychiatry, psychotherapy, substance use treatment, and Enhanced Care Management (ECM). The preferred candidate is a Family Nurse Practitioner (FNP); experienced Physician Assistants (PA-C) with significant outpatient family medicine experience are encouraged to apply and will receive equal consideration. Essential Functions Provide preventive, acute, and chronic disease management. Diagnose, treat, prescribe, and coordinate care. Collaborate with psychiatrists, therapists, ECM staff, and medical assistants. Perform wellness visits, chronic disease management, and preventive screenings. Coordinate specialty referrals and transitions of care. Complete timely EHR documentation and participate in quality improvement and HEDIS initiatives.
California licensure as an FNP or PA-C in good standing. DEA registration and furnishing authority as applicable. Current BLS certification. Eligible for credentialing with Medicare, Medi-Cal, CenCal Health, and commercial payers.
Family Nurse Practitioner strongly preferred. Physician Assistants with substantial outpatient primary care experience receive equal consideration. Experience in integrated behavioral health, community health, or value-based care. Bilingual English/Spanish preferred.
