- The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity.
- Responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care.
- Must demonstrate competence in dealing with age-related needs of the specific patient populations served in nursing unit.
- Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Laurel Heights and facility goals and strategic plans.
- What you will do Demonstrates ability to assess patient clinical and psychosocial needs by applying standards of practice as it relates to the specialty or patient population.
- Demonstrates nursing unit specific competencies and equipment training, see checklists.
- Partners with patients and their family to develop, implement and evaluate a plan of care, treatments and procedures that contribute to physiological and psychosocial outcomes of the patient.
- Incorporates evidence-based standards of practice and collaborates with interdisciplinary team in care coordination.
- Facilitates safe patient transition to next level of care as part of plan of care.
- Demonstrates sense of urgency to facilitate patient throughout to ensure patient receives consistent, competent and ethical care.
- Focuses on quality outcomes of patient when addressing service needs to ensure evidence-based quality care of patient.
- Demonstrates understanding of diverse patient care populations.
- Modifies education method to ensure patient/family understanding.
- Assumes responsibility to coordinate and deliver individualized patient education and discharge specific education to family/caregivers.
- Evaluates for comprehension and documents.
- Accurately completes all required clinical and quality documentation consistently per Laurel Heights standards and regulatory requirements.
- Delegates and prioritizes tasks appropriately to facilitate quality patient care and service excellence to meet the patient's care needs.
- Recognizes and responds to emergency situations.
- Evaluates resources and support needed and uses systems to ensure needs are met.
