Whether providing direct client care, developing individualized treatment plans, you’ll play a key role in enhancing clients' independence, safety, and overall quality of life.

  • Therapist, you will: Provide Direct Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders. Maintain high standards of care while adapting to different environments and client needs. Maintain Accurate Client Records and Billing – Document all services accurately and timely using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines. Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery. Ensure Regulatory Compliance – Complete and document supervisory visits with OTAs and CNAs in accordance with state and regulatory guidelines. Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
  • This job also requires: Current Occupational Therapist (OT) license in the state of Minnesota and North Dakota. CPR certification required. Valid driver’s license and access to an insured personal vehicle. Willingness to float across regions as needed. Ability to pass state mandated background check. Strong clinical skills in occupational rehabilitation and skilled therapy services. Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members. Experience in Home Health, acute care, or rehabilitation settings preferred. Physical capability to perform all job duties.

This role includes documenting collaboration with COTAs and performing supervisory visits with HHAs but does not include formal supervisory responsibilities. Requirements

Must hold a current Occupational Therapist license in Minnesota and North Dakota, along with CPR certification and a valid driver's license. Experience in home health, acute care, or rehabilitation settings is preferred.
