What You’ll Do Provide skilled physical therapy services to home health patients across the Omaha metro area Develop individualized treatment plans focused on mobility, strength, safety, and independence Partner closely with nurses, providers, caregivers, and interdisciplinary teams to support patient goals Complete timely documentation and maintain compliance with all regulatory requirements Educate patients and families on exercises, safety, and continued care plans Build trusted relationships with patients while delivering compassionate, high-quality care What We’re Looking For Licensed Physical Therapist in the state of Nebraska (or eligible for licensure) Previous home health experience strongly preferred Minimum of 1+ years of PT experience required Strong clinical judgment, communication, and organizational skills Ability to work independently while staying connected with the care team Reliable transportation and valid driver’s license Why Hillcrest?
