Social Worker | Full-Time Non-Exempt Rate of Pay

$28.95 to $33.00/hour based on degree, licensure, and experience

Job Description

As a Social Worker, you'll be at the forefront of building meaningful relationships with our residents, their families, and designated representatives. You'll be their go-to person during important transitions between different levels of residency or other life changes, offering guidance, support, and a friendly ear to listen. But that's not all—your role as a valued member of our interdisciplinary team will also involve contributing to Person-Centered Care Planning and, with our Director of Social Work, ensure we meet all regulatory requirements for our 48-bed skilled nursing department within our vibrant life plan community. You’ll assist with quality assurance initiatives, facilitate various resident support groups and staff education as needed and act as a liaison to the Long-Term Care Ombudsman program.

Qualifications

  • Masters or Bachelors degree in social work, with 2 years of experience
  • Experience in long-term care, health care, end-of-life care, or gerontology
  • Preferred, but not required, LMSW
  • Excellent interpersonal and time management skills
  • Ability to collaborate effectively within an interdisciplinary team
  • Proficiency in computer programs such as Word, Excel, Outlook, etc.
  • Familiarity with Electronic Medical Record Software

Requirements

Requires a Bachelor's or Master's degree in social work with at least 2 years of experience. Experience in long-term care, gerontology, or end-of-life care is required, and an LMSW is preferred.
