Salinas 93906 - 16 year old young lady with a primary diagnosis of Wolf-Hirschhorn syndrome, CKD stage 2 and hx of cleft palate.
Has a hx of seizure disorder, OSA, conductive hearing loss and is developmentally delayed.
Patient goes to school M-F.
CNA's responsibilities include providing all ADLs to patient while at home after school which includes preparing and managing feedings, changing patient and bathing patient.
Patient is able to ambulate but requires wheelchair for long distances.
Parents needing full time M-F 2pm-8pm which equates to 30 hours per week.
