About the role

The Family Nurse Practitioner provides comprehensive primary care to patients of all ages through office, mobile, and home visits. Responsibilities include diagnosing acute and chronic conditions, managing geriatric health, and coordinating care with specialists and community resources.

Position Title

Family Nurse Practitioner (FNP-C or FNP-BC) REPORTING RELATIONSHIP: Chief Medical Officer STATUS: Non-Exempt SUMMARY: The Family Nurse Practitioner (FNP-C or FNP-BC) maintains an independent clinical practice under the direct supervision of the Chief Medical Officer, as a member of the SHHC’s clinical team, responsible for office visits, mobile medical unit visits, and home visits as appropriate to meet patient needs. It is expected that the Nurse Practitioner will care for patients of all ages, from newborn through geriatric in all SHHC locations. In addition to a general practice, Nurse Practitioner may provide routine prenatal and gynecological care to SHHC patients, not including deliveries, and will work with SHHC’s contract OB/GYN and prenatal team to ensure delivery of highest quality care to patients enrolled in the prenatal program. The Nurse Practitioner is a key member of the FQHC care team working to provide quality comprehensive services to patients and will work collaboratively with staff to provide team-based coordinated care. The ideal candidate has strong clinical experience providing comprehensive primary care services across the lifespan, with an emphasis on diverse adult, geriatric, and family medicine populations; and is passionate about serving medically underserved, diverse communities in a mission-driven community health setting. PRIMARY RESPONSIBILITIES: Conduct patient assessments, physical exams, diagnose acute and chronic conditions, and develop individualized treatment plans Orders or executes various tests, analyses, and diagnostic images to provide information on patient’s condition Analyzes reports and findings of tests and of examination, and diagnoses condition Manage chronic disease states common in geriatric populations (diabetes, hypertension, dementia, polypharmacy, falls risk etc.) Inoculates and vaccinates patients to immunize from communicable diseases Advises patients concerning diet, hygiene, and methods for prevention of disease Ensures that medical care is completely and accurately documented in the medical record Coordinate care with specialists, behavioral health providers, social workers, and community resources Performs minor surgery and other procedures commensurate with surgical competency and delineated privileges Participates in quality improvement initiatives, HRSA/UDS reporting requirements Maintains and monitors patient confidentiality Provides care on SHHC’s mobile medical unit on a schedule determined by SHHC’s Chief Medical Officer Takes rotating after hours call with other clinicians at the direction of the SHHC Chief Medical Officer Prepares and signs all forms and charts, including all charting in electronic health record, needed for patient care and billing within three working days Makes house and emergency calls to attend to patients unable to visit office or clinic at the direction of the Chief Medical Officer Abides by the policies and procedures of the Squirrel Hill Health Center and acts as a positive representative of the center in the community Ensure culturally competent, trauma-informed care for diverse and underserved populations MINIMUM QUALIFICATIONS, TRAINING AND EXPERIENCE Advanced degree and certification from an accredited Nurse Practitioner program A license without restriction to practice as an Advanced Practice Registered Nurse (APRN) in Pennsylvania Active DEA registration (or eligibility to obtain) Participation in the Medicare and Medicaid programs Enrollment in required managed care and insurance contracts Demonstrated sound problem-solving and decision-making ability Demonstrated skill in planning and organizing work, managing time, and meeting commitments Demonstrated well-developed interpersonal skills to interact in sensitive and/or complex situations with a variety of people; expertise with multicultural populations Demonstrated very high level of communication skills to effectively communicate verbally and in writing Ability to work independently, collaboratively, and calmly in fast paced mission-driven environment Ability to represent organization in a professional manner Current BLS or ACLS certification Experience with EHR systems Flexibility and willingness to work some evenings and in all SHHC locations Knowledge of a second language preferred Prior experience in a community health setting preferred All CDC recommended vaccinations, including COVID-19, required.

Requirements

Candidates must hold an advanced degree and certification from an accredited Nurse Practitioner program and a valid APRN license in Pennsylvania. Required qualifications include active DEA registration, BLS or ACLS certification, and the ability to work in a fast-paced, mission-driven community health setting.
